Athletic Forms
Athletes who will begin attending Washington High School and Junior High School in the fall of 2022 MUST have a physical and online forms on file in the athletic office. Athletes will only be allowed to participate if the physical is turned in and online forms are completed in Eventlink.
Physical forms are also available in the athletic office.

The physical may be turned into the athletic office, emailed to either of the above contacts, uploaded to Eventlink when you complete the online forms, or dropped in the dropbox outside of Gate 6 at the High School.
Requirements: Both must be completed to participate.
- Physical-filled out by doctor and parents with all signatures completed.
- Online forms completed on Eventlink.
