$20 per physical - cash/check only.
You must have your health history (pages 1-2) and signature page (page 5) completed by a parent or we cannot see you! Forms must be turned in to the athletic office by May 9th. Blank forms are available in the Athletic office, JH office, and Upper Elementary office.
