Important Information for Student Athletes

If your child is planning on participating in athletics for the 2023-24 school year, they must have a physical completed after April 1, 2023.

The CORE Center will provide Sports Physicals on May 23, 2023, from 3:00 - 6:00 pm.

The physical and Eventlink athletic forms are required to be completed to participate in any summer open gyms, open fields, or official practices.

A computer or iPad is recommended when completing the online forms. It is very difficult to do on a phone.

Please drop the physical forms off at the High School athletic office. During summer hours, forms may be placed in the drop box at Gate 6 if no one is available.
